Parental Consent:

By signing this I consent to my child being a part of their club team. I understand the expectations for being a part of this club, and agree to support my child in what they need to grow as a player. I agree to pay the club dues in full, and by the set date. If my player loses their designated ball, I understand I am financially responsible for replacing it.

I agree to be respectful and communicate my needs effectively. I will be responsible for my designated ball and understand the consequences if I should lose it.
